In the wake of the implementation void following the presentation of the Romanow Report, this book proposes a practical, community-oriented, public-private action plan for the Canadian health care system. The book argues for adoption of the principles of Patient Health Management, a method of looking at health care that puts the patient at the center of the health system and has been proven to result in better prescription by doctors as well as better compliance by patients. Adopting a Patient Health Management model will also translate into better patient access to products and services, in addition to better value in terms of clinical and economic outcomes for the system. Key features of the Patient Health Management model include closing care gaps between best care and usual care through community-based partnerships and repeated measurement and feedback of practices and outcomes, generating a continuous quality improvement loop that ensures more appropriate prescribing and compliance. The book will explore the principles and the evolution of the Patient Health Management model and provide case study examples, population-based evidence and first person testimonials by practitioners, patients and policy makers as to the advantages of large scale adoption of the Patient Health Management model.
